Eligibility Requirements
Affinity Angels Home Estates provides adult shared living and independent community homes for residents who meet specifc care, health, and living criteria.
General Resident Profile
Residents are primarily single adults, seniors, and veterans who are independent or semi-independent in their daily living. The homes are intended for adults who need housing and supportive services but do not require intensive medical care. The environment supports individuals seeking stability, community, and a path toward greater independence
Basic Eligibility Requirements
- Adults, primarily seniors
- Able to live independently or with minimal assistance
- Relatively stable physical and mental health
- No children permitted in shared community homes
- Monthly payment required via ACH draft or ZELLE
- Completion of a resident assessment process
Health & Behavioral Exclusions
Affinity Angels Home Estates does not accept individual who are:
- A significant fall risk
- Indoor smokers
- Exhibiting aggressive behaviors
- In need of continuous skilled nursing care
- Requiring 24/7 awake monitoring
- Bedridden
- Requiring two-person transfers
Assessment Process
Needs Assessment
Initial intake conducted by phone or video. In-person meetings at the resident’s home, hospital, or rehabilitation facility are preferred.
Health & Functional Evaluation
Review of medical history and ability to perform Activities of Daily Livings (ADLs).
Home Safety Review
Fall-risk or safety evaluations may be completed to ensure a safe living environment.
Client Skill Matching
Evaluation used to match resident needs with the experience and training of the live-in or drop-in house manager.
Eligibility Review Criteria
During the assessment, the following areas are evaluated:
- Level of independence
- Mobility and fall risk
- Medication management needs
- Behavioral suitability for shared living
- Clinical review by nurse practitioners or psychiatrists when required
